As you can see, our partners include community providers, mental health providers and local authorities, which provide social care.
Equally importantly, our work involves you. We have a group of community ambassadors – ordinary people who are our eyes and ears - helping the wider community to understand what we are doing, and why we are doing it.

Happy, healthy, at home spans two counties – Hampshire and Surrey. This presents us with some challenges but also opportunities to take the best from each and apply it to our programme.
Our Steering Group meets every three weeks to assess progress made and plan for the future. We have had visits from the national team from NHS England, which have been very positive about the work we are doing and our future plans for improving health and care for our local population.
