Right information at the right time

One of things patients tell us is that they wish they only had to tell their story once. This is particularly true for patients who have complex conditions, or for carers who talk to health and social care workers.

A key enabler to integrated care is the ability for professionals to see the right information about their patients, when they need to.

Already the team has achieved a major step towards this with the introduction of a system call EMIS Viewer into the A&E department at Frimley Park Hospital. This system means that A&E staff are able to see a patient's GP records as soon as the patient arrives (and sometimes while they’re still in the ambulance). This means A&E staff have real-time access to information like medication lists and allergies.

Previously it could take up to 72 hours for this information to be shared, which meant patients were being admitted while this happened—not great for patients, or for the hospital.
